Community Inclusion Planning Coordinator (Full-time Hybrid, North Carolina Based) (Personal Services)
The Community Inclusion Planning Coordinator oversees the Community Inclusion Planning Meeting (CIPM) program in Durham, Johnston, Wake, Cumberland, Mecklenburg, and Orange counties to promote growth of the program and ensure fidelity to the model.
This position will allow the successful candidate to work a primarily remote schedule. Two days a week working onsite is required as well as working some evenings and weekends. This position will require extensive travel in Wake, Durham, Orange, Cumberland, Johnson, Mecklenburg, and Harnett counties for meetings and events.
Essential Functions & Duties
Provide oversight of the CIPM process for assigned areas
Conduct CIPM meetings and develop teams to meet the needs of the individual served.
Collaborate with community partners to address critical needs of individuals served
Collaborate with internal departments to ensure continuity of care
Knowledge, Skills & Abilities
Minimum Requirements
Bachelor's degree in a Human Services field and five (5) years of progressive experience in the field of Health Care or Human Services;
Or
Master's degree in a Human Services field and three (3) years of experience in the field of Health Care or Human Services.

Salary Range
$28.12 - $35.85/ Hourly
Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity.
