Behavioral Health Counselor - Data Manager Gwinnett Court Services (Personal Services)
View Point Health's Court Services provides group, family, and individual education, skills-based training and supportive counseling to consumers and their families/significant others. Designs counseling/education services that meet the individual needs of the consumer. Educates families/significant others on the addiction and recovery process.Assesses consumers treatment needs and assists him/her to develop appropriate individual treatment goals.
Meets with consumers at established intervals to discuss progress toward goals. With input from the clinical coordinator as needed, identifies unmet needs, reviews available resources, and suggests modifications to the treatment plan. Writes clear, concise, complete, grammatically correct, and timely progress notes that relate directly to the treatment outcomes described in the service plan. Completes discharge summaries and other documentation and reports accurately and timely.
Serves as case manager to facilitate and ensure communication, linkage, and continuity of care between consumers, physicians, other organizational components, community resources and service providers. Participates in case management and discharge planning meetings to share information and facilitate referrals to other services and agencies. Establishes linkages and coordination of services with community agencies. Develops and maintains cooperative working relationships with community service providers. Informs consumers and families of the availability of community advocacy and support groups. May serve as liaison and consumer advocate to local human service agencies and community resource support groups.
Duties & Responsibilities:
Provide group, individual and family sessions to court participants.Provide documentation on all services provided in View Point Health medical record and court case management system.Provide case management services and linkage to court participants.Function as part of the Drug, DUI & FTC Court teams.Attend court and staffing for Drug, DUI & FTC Court as needed.Attend training as needed for court evidence-based curriculum.Complete GPRA Assessments for SAMHSA grant and enter into SPARS systemAssesses consumers treatment needs and assists him/her to develop appropriate individual treatment goals.Meets with consumers at established intervals to discuss progress toward goals. Minimum Qualifications:
Completion of a Bachelors degree in a related field.Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities.
Preferred Qualifications:
Experience providing groups Experience providing individual and family sessions Experience working with clients involved in the criminal justice system Experience providing case management Experience with Excel, spreadsheets, etc. Potential to become licensed or certified in the future
Requirements/Competencies:
Must be able to lift 20 pounds.Requires long periods of sitting/standing.Must have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy.Candidates for selection must pass a criminal background check (including fingerprinting).Must pass a pre-employment drug screen and subject to random drug screens.Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's license.Other duties as assigned.
