Job Description:
Willow Creek Healthcare Center, a reputable Skilled Nursing Facility in Clovis, CA, is seeking a compassionate and dedicated Admissions Nurse LVN to join our team on a part-time per diem basis, working weekends only. This position is responsible for facilitating the admission process for new residents, ensuring they receive exceptional care and attention from the moment they enter our facility.
Key Responsibilities:
- Conduct comprehensive assessments of new admissions, including medical histories, physical exams, and gathering necessary documentation
- Coordinate and collaborate with interdisciplinary team members to develop and implement personalized care plans for new residents
- Communicate effectively with patients, families, and the admissions team to ensure a smooth and welcoming transition into the facility
- Administer medications and treatments as prescribed during the admission process
- Provide support and education to residents and their families about the services and care offered at Willow Creek Healthcare Center
- Maintain accurate, up-to-date patient records in compliance with facility and regulatory standards
- Ensure a smooth handoff of the resident to the nursing staff after admission
- Adhere to all facility policies, procedures, and compliance requirements
Qualifications:
- Valid LVN license in California
- Previous experience in skilled nursing, hospital, or long-term care settings is preferred
- Strong communication and interpersonal skills
- Compassionate, empathetic, and patient-centered approach to care
- Ability to work independently and as part of a team
- Must be available to work weekends (Saturday and Sunday)
- Knowledge of admissions processes and regulatory compliance is a plus
