MDS Coordinator (Healthcare)

Wage Range: $45/hr-$55/hr

Reports To: Director of Nursing

Job Summary: The MDS (Minimum Data Set) Coordinator is responsible for overseeing the MDS assessment process in long-term care facilities. This role ensures compliance with federal and state regulations, maintains accurate patient records, and coordinates with interdisciplinary teams to develop and implement individualized care plans.

Key Responsibilities:

• Assessment and Documentation:
  
  • Complete and update MDS forms for all residents in the facility.
  • Perform clinical assessments to gather comprehensive information on residents' health status.
  • Ensure accurate and timely submission of MDS data to maintain compliance with Medicare and Medicaid requirements.
• Care Planning:
  
  • Develop and update individualized care plans based on MDS assessments.
  • Coordinate with nursing staff, physicians, and other healthcare professionals to implement care plans.
  • Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
• Compliance and Quality Assurance:
  
  • Ensure all MDS documentation meets federal and state regulations.
  • Participate in quality assurance and performance improvement activities.
  • Conduct audits and reviews of MDS documentation to ensure accuracy and completeness.
• Communication and Collaboration:
  
  • Facilitate interdisciplinary team meetings to discuss resident care and progress.
  • Communicate with residents and their families to explain care plans and address concerns.
  • Serve as a resource for staff regarding MDS processes and regulations.

Qualifications:

• Education:
  
  • Registered Nurse (RN) preferred or Licensed Practical Nurse (LPN) with a valid state license.
  • Certification in MDS (e.g., RAC-CT) preferred.
• Experience:
  
  • Minimum of 2 years of experience in a long-term care setting.
  • Previous experience with MDS assessments and care planning.
• Skills:
  
  • Strong knowledge of MDS 3.0 and federal/state regulations.
  • Excellent organizational and time management skills.
  • Effective communication and interpersonal skills.
  • Proficiency in electronic health records (EHR) systems.

Work Environment:

• Primarily works in a long-term care facility.
• May require occasional evening or weekend shifts to meet the needs of residents.
