LPN - Personal Care (Healthcare)
This opening requires an active Practical Nurse License from the Commonwealth of Pennsylvania.
The LPN assesses, plans and implements the nursing care of residents within the personal care setting. Responsible for medication pass, charting, providing care in accordance to physician orders, ensuring continuity of care of the residents between shifts by providing direct care, as well as supervising the care given by CNAs and supportive staff members. The LPN at Homeland is to also maintain a role as resident advocate with a focus on the facility's mission, goals, and objectives associated to high quality care.

REQUIREMENTS:
Education and/or Experience:
Associate's degree (A. A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
Computer Skills:
Experience and familiarity with computerized charting with programs such as Caretracker, Computerized Medication Cart, and Electronic Medical Record is helpful, but not required. Must be willing to be trained on computerized charting and documentation as it is implemented in the facility.
Certificates, Licenses, Registrations:
Valid and Current Practical Nurse License from the State Of Pennsylvania Licensure Board
Other Skills and Abilities:
Prior experience with Geriatric residents
Compassion and Caring
Team Player that is willing to assist in the completion of the overall goal for the benefit of our residents
Willingness to assist others to accomplish the goals of the facility
