Nemours is seeking a Registered Nurse for out Preventable Heart Disease Specialty Nurse role here in Wilmington, DE! The Registered Nurse is a clinically competent practitioner for children and teens with obesity, hypertension, hyperlipidemia and family history of premature coronary artery disease. You will work closely within a team of multiple providers including physicians, advanced practice providers, and social worker. The RN is responsible for assisting with all aspects of care, including navigating, and providing education to assist the patient through the health care process assuring that continuity of care is promoted in all patient care settings. Collaborate with the physicians, other healthcare team members, and the patient/family to maintain a plan to deliver high quality healthcare in the outpatient setting and in the home environment.
Responsibilities:
Send lab orders, instructions and provider requested communications before new or follow-up appointments.
Communicate lab results to families within 24-48 hours of receipt in EPIC.
Respond to messages from patients and family members.
Create and contribute to the development educational materials for patients and families.
Provide teaching to patients and families.
Proactively develops and enhances processes to improve clinical experience for referred patients and the referring physician.
Assist and complete all prior authorizations for GLP1RA prescriptions.
Proactively develop quality improvement initiatives but QI requested for the following: body mass index assessment, blood pressure assessment, cholesterol counseling, nutrition counseling, blood pressure management, GLP1RA education.
Provides clinical assessment expertise to scheduling parameters - timing of visit, appropriate provider, etc. Improves access to care by assuring efficient and effective coordination between visits.
Assist with coordinating cross specialty care, including care provided by Weight Management/Healthy Choices clinic, Endocrinology, Nephrology (ambulatory blood pressure monitoring), Hepatology, nutrition, and cardiac rehab/fitness program.
Coordinates multidisciplinary/comprehensive clinics, assisting with clinic flow and alerting providers to patient needs.
Supports the work of research and serve as a critical interface.
Coordinates and delegates prior authorizations and peer-to-peer.
Participates in multi-disciplinary team discussions in relation to the individualized patient care plan. Bridging gaps in care and supporting care coordination for patients with highly complex care needs and serves as a liaison between the patient/caregiver and provider when necessary.
Evaluates the patient/family response to care on an ongoing basis. Tracks patient outcomes and relevant data using information systems and pathways. Maintains any registries as needed.
Promotes and provides patient, family, staff, and community education using clinical expertise and knowledge base in person, by phone, or via telehealth.
Triages patient phone calls, and communications, through approved mediums, and provides educations, treatment plans in line with established plan from clinical team.
Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee's supervisor.
